Join us on November 14, 2026, from 1:00–3:00 PM at the Lee Road Library in Cleveland Heights for a health screening, nutrition consultation, and guidance on insurance options. Hometown Health is committed to community engagement and addressing social isolation through supportive community-based care. Learn about primary care access and critical updates regarding Ohio Medicaid work requirements and eligibility renewals starting in January 2027.
Cardiovascular Health
Mark Rodney, PA-C will offer a health screening including blood pressure check and other vitals and a brief assessment of your cardiovascular risk factors. He will discuss cholesterol and other laboratory measures and imaging tests that help assess your risk of heart attack, stroke, and other cardiovascular conditions.
Nutrition
Meet with a registered dietician to discuss how dietary choices can help reduce your blood pressure and cholesterol and potentially reduce your need for medications.
Insurance
A licensed insurance counselor will be available to answer questions and offer guidance about what insurance options best meet your goals.Â
